The med-surG nurse practice framework

Stop Guessing Your Way Through Med-Surg Orientation

The Med-Surg Nurse Practice Framework is a structured system that helps new med-surg nurses organize their thinking, navigate orientation successfully, and feel confident on the floor in weeks, not months.

Does this sound like you?

You graduated from nursing school. Maybe you even passed the NCLEX in 75 questions. Maybe you've worked as a nurse in outpatient, psych, or a nursing home. And now, you're starting a new job in med-surg and feel shell-shocked at the pace and expectations.

You want to be seen as a high performer. You thought you were a high performer... and now you're questioning whether you can even do this.

You never anticipated that...

Orientation is inconsistent and depends heavily on who your preceptor is.

You never learned how to think through an entire shift with a full patient load.

Everyone assumes you'll pick it up or someone else told you, but no one explains the big picture.

You're left guessing a lot, overworking in the wrong places, and wondering if you're falling behind.

Here's the truth:

It doesn't have to be this hard or confusing

It doesn't take decades of experience to become a safe and competent med-surg nurse. The experienced nurses working alongside you use a mental framework to organize care, prioritize risk, and make decisions consistently.

Once you understand that framework, your orientation stops feeling random and becomes less dependent on your preceptor, and your confidence grows quickly.

This program is for new grads starting on med-surg, experienced nurses transitioning to med-surg from another area, or anyone who feels like orientation is happening to them instead of for them.

Meet your teacher

Hi, I'm Kati!

I’m a RN with bedside experience across cardiac med-surg, stepdown, and neuro ICU, and a Master’s degree in Nursing Education. I have been a bedside nurse, charge nurse, and preceptor. I’m also the founder of FreshRN, an education company that has supported thousands of nurses since 2015.

This course is built directly from that experience, not theory or recycled NCLEX content. The original version of this program, published in 2020, was developed alongside two nationally certified medical-surgical nurses, and it has since been refined based on real feedback from hundreds of new grads navigating med-surg units in the real world.

I created the resource I wish I had when I started in 2010, something that makes the expectations of med-surg visible, structured, and manageable instead of vague and overwhelming.

On the fence?

Here's how to know if the Med-Surg Nurse Practice Framework is right for you:

This is for you if...

You're new to med-surg nursing

You are caring for adult patients in the med-surg environment in the United States

You have a new preceptor every shift

You want clarity, not textbook answers

You feel overwhelmed or unsure how to prioritize

You want ownership over your orientation and not to be at the mercy of whichever preceptor you have each shift

This is not for you if...

You're looking for a skills certification

You want unit-specific policies

You expect orientation to be replaced

You're unwilling to apply what you learn

You want med-surg NCLEX-prep material

You want support for outpatient nursing care, ICU, emergency department, or pediatrics

You want this to replace your med-surg clinical rotation

Structure for Your Med-Surg Shifts When Everything Feels Unclear

Confidence doesn't come from guessing more. It comes from having a framework that guides your thinking at the bedside. This framework helps you understand what's expected of you, what deserves your attention, and what doesn't. So you can ask better questions, set realistic expectations for yourself, and stop burning energy on the wrong things.

Frequently asked questions

  • Does this program come with contact hours?
No, it does not. We chose not to grant contact hours to keep the course cost low for new graduate nurses.
  • What if I don't like the course?
We would never want you to be unhappy with your purchase! Simply email us at contact@freshrn.com and let us know. To qualify for a refund, less than 50% of the course must be completed, and the refund must be requested AND issued within 14 days of the original purchase. Please allow 1-2 business days for processing refunds.
  • Do I need to watch everything in order?
No. While the framework builds logically, you can jump to the areas most relevant to what you’re experiencing during orientation.
  • Are there any limits in logins or or video watches?
No, once you purchase you have lifetime access and can watch videos as many times as you'd like. There is no limit.
  • Can nursing students or experienced nurses enroll?
While this is designed for new med-surg nurses, there are no limits on who can enroll. Students may find the content helpful in preparing for clinical rotations on med-surg units, but it will not provide any NCLEX-prep material. Experienced RNs are welcome to enroll and may find the content helpful in refining their shift management.
  • Will this make orientation easy?
Orientation will still be challenging. This program won’t remove the learning curve, but it will help you navigate it with more clarity, structure, and confidence.
  • Is this replacing my hospital’s training?
No. This program is meant to support and enhance your orientation, not replace employer training or policies.
  • I'm in a med-surg specialty. Will this work?
If you're working in med-surg/tele, med-surg/ortho, med-surg/cardiac, med-surg/stroke, this course will help. While we don't go into speciality specifics like cardiac, ortho, or neuro in detail, the overall framework for orientation, growth, time management, prioritization, and more is applicable. We have specialty courses available; however, I highly recommend establishing a strong med-surg foundation before those specialized courses.
